Campaign Disclosure Statements (Forms 410, 460, 470, 501, etc.)

Effective January 1, 2021, AB 2151 requires a local government agency to post on its internet website, within 72 hours of the applicable filing deadline, a copy of any specified statements, reports, or other documents filed with that agency in paper format and must be made available for four years from the date of the election associated with the filing. Per AB 2151, copies of Campaign Disclosure Statements filed by the City Council may be reviewed on this page. Please note that certain personal information for statements that are posted electronically (i.e., online or by email) are required to be redacted. Unredacted copies of campaign statements are available for review in the City Clerk's Office at 12 Journey, Suite 100, Aliso Viejo, CA 92656 or can be requested via email at city-clerk@avcity.org

Statement of Economic Interests (Form 700)

The California Political Reform Act (Government Code Sections 81000-91014) requires many public officials and employees to file financial disclosure reports known as Statements of Economic Interest (Form 700) when assuming office, annually, and when leaving office. Cities are also required to adopt and implement a Conflict of Interest Code. The City Clerk, as Filing Officer, administers the Conflict of Interest Code at the local level.

The following public officials are identified in Government Code Section 87200 and file Statements of Economic Interest with the City Clerk’s Office:

  • City Attorney (Scott Smith)
  • City Council (Tiffany Ackley, Ross Chun, Max Duncan, Richard Hurt, and Mike Munzing)
  • City Manager (David Doyle)
  • Finance Director/Treasurer (Ann Eifert)

Agency Report of Ceremonial Role Events and Ticket/Pass Distributions (Form 802)

Title 2 of the California Code of Regulations, Section 18944.1 requires that any distribution of tickets or passes to or at the request of City officials, employees and consultants must be made pursuant to a duly adopted written policy. Agency Report of Public Official Appointments (Form 806)

Title 2 of the California Code of Regulations, Section 18702.5 requires the completion of Form 806 to report additional compensation that officials receive when appointing themselves to positions on committees, boards or commissions, etc. Each agency must post on its website a single Form 806 which lists all the paid appointed positions.
